When you complete the
enrolment form you may submit via email to
enrol@clja.com.au or print, sign and
mail the form to:

NOTE:
Enrolments will not be processed until, at
least the first payment of the designated
payment plan is provided.
Payment may be made using the

advise staff that you are direct depositing
payment or sending money order or cheque.
IMPORTANT:
